Save time (and postage!). Download and fill out an application and drop it off at one of our agencies near you or at one of our borough-wide events! Please do not mail completed applications to our office. Applications mailed to our office for first-time applicants will not be processed.
When handing in a completed application, your child must accompany you. Please also remember to bring a copy of your child's health insurance card and copies of his or her recent immunizations.
