Join Us at a Registration Event!
In order to make it easier for you to register your child for our programs, we hold several free events throughout the spring and early summer. If you would like to register your child(ren) at one of these events, please be sure to bring the following information:
1. Medical Insurance or Medicaid Card
2. Public Assistance Information (if applicable)
3. Copy of your child's physical examination signed by a physician
4. Three emergency contact names and phone numbers
Additionally you may download our application and bring the completed forms with you to the event. Don't worry if you cannot fill out the entire application. Our experienced staff will be there to assist you. In addition to reading the list below (which will be continually updated throughout the spring), please check for an agency in your community who provides ongoing registration opportunities.
